The best way to do it is to contact a lawyer that practices in the area of estate planning and elder law. All right? They'll ask you particular questions. Your name, who do you want in charge? They should give you options on the powers that you want to include in the power of attorney. While they may be important to have there, why they might not be, advise you. They draft the document, you then sign it, get it notarized, you then have a power of attorney.
